Write-up: Patient age 13 yr. 4 months seen for follow-up on acne and allergic rhinitis. Given HPV2 - sat in room for 5 minutes then on way to lobby stopped in bathroom - fainted while washing hands and again en route to exam room. Full recovery in 30 minutes.

Write-up: Dr. gave 1st HPV 9-2-10. Within a few days, pt. passed out, in 2 weeks passed out again. Spit up blood, bad headaches, stomach ache, hives. Dr. gave her HPV shot; recommended booster in 1 month. She passed out a week later, had panic attack, bad headaches, stomach and pelvic pain, legs cramping up at night. Waking crying, couldn''t walk at first.

Write-up: This case was reported by a physician, via a sales representative, and described the occurrence of swelling injection site in a 22-year-old female subject who was vaccinated with CERVARIX (GlaxoSmithKline). On 20 August 2012 in the "pm", the subject received 3rd dose of CERVARIX .5 ml, unknown route, right arm). On 20 August 2012 in the "pm", less than one day after vaccination with CERVARIX, the subject experienced swelling injection site and injection site redness. The swelling at vaccination site was "the size of a golf ball". At the time of reporting the outcome of the events was unspecified. Follow-up information was received on 18 September 2012 via healthcare professional who provided no additional information.

Write-up: This retrospective pregnancy case was reported by a pharmacist and described the occurrence of vaccine exposure during pregnancy in a 21-year-old female subject who was vaccinated with CERVARIX (GlaxoSmithKline) during pregnancy. The subject''s medical history included previous pregnancies include two full term. On 26 September 2012 the subject received 3rd dose of CERVARIX (.5 ml, intramuscular, unknown). Her last menstrual period was on 18 August 2012 and estimated date of delivery was not provided. The subject used no contraception and the pregnancy was by normal conception. On 26 September 2012, at the time of vaccination with CERVARIX, the subject experienced vaccine exposure during pregnancy. The subject received her 3rd dose of CERVARIX on 26 September 2012. Later that same day, the subject received a positive pregnancy test. The subject decided to terminate the pregnancy for reasons unrelated to CERVARIX vaccination [elective aborted pregnancy]. At the time of the report no additional information was available. The outcome of the pregnancy was elective termination with no apparent congenital abnormality.

Write-up: Subject was enrolled in Study from 5/18/2011 to 12/20/2011, and received 2 doses of CERVARIX and ENGERIX B vaccines. Per protocol, patient received negative urine pregnancy tests prior to each vaccination. Following closeout from Study, patient continued in the Extended Follow-up Study (since 11/25/2010). At study visit on 3/4/2013, patient reported she took a urine pregnancy test on 1/15/2012 that was positive and later the same day started spotting and contractions. Patient went to the ER and was told by a doctor she had had a complete miscarriage. Patient had IV solution (not specified), and was released the same day with rest indicated. Patient reported she was fully recovered by 1/22/2012.
