Preexisting Conditions: No allergies were reported. The pt had ulcerative colitis 30 years ago. One year ago, her liver function tests were normal.
Allergies:
Diagnostic Lab Data: CT-scan of the abdomen showed obstruction of the biliary system, but no evidence of stones. She then underwent ERCP(endoscopic retrograde cholangiopancreatography) which showed a small adenocarcinoma of the ampulla which was subsequently removed with a whipple procedure.

Write-up: This is a spontaneous report from a physician referring to a 60 year old female who, 7 days post vax, on 3/20/2000, experienced elevated liver function tests and intense pruritis. A doctor visit was required. No treatment was given. Havrix was discontinued. Her liver function continued to deteriorate. The most recent information, received on 3/24/2000, reports the vaccinee is now well and presumable disease free since all her pathology was negative. She is also symptom free and her itching and liver function tests have returned to normal.

Write-up: On 5/13/00, the pt developed generalized fatigue and weakness, progressing to weakness and numbness in upper and lower extremities. Continued increasing muscle weakness with difficulty walking on 5/16/00. Sought medical advise on 5/17/00. Admitted to hospital and dxed with Guillain-Barre Syndrome based on spinal tap results and nerve conduction studies. Treated with immunoglobulins. Follow up 07/27/2000: "Patient final diagnosis Gullian-Barre Syndrome; was diagnosed by neurologist. Patient experienced other illnesses within 4 weeks before onset of GBS syndrome; Patient experienced or has other previous illness or medical condition before the onset of GBS symdrome such as Ulceration Colitis in 1992, no flareups within past year. Patient received influenza vaccinations previously."

Write-up: It was reported that a female pt who, post vax, reportedly, her hamstring of her right leg began to hurt within days of the vaccinations. The pain was so intense that the pt was unable to walk. Pt was hospitalized for 4 days and was dx''d with arthritis in the hip. Pt is on crutches and experiences muscle pain with shooting pains down her leg. As long as pressure is off leg, the pt only experiences muscle weakness, no pain. Further information has been requested. Follow-up dated 7/11/00, it was reported that the pt began experiencing pain in her right leg on 5/10/00. The pt was hospitalized from 6/1/00 to 6/4/00 for further evaluation. A x-ray of the right hip was positive for arthritis. Additional pt and vaccine administrator info were also provided. Follow-up 05/15/02: During internal auditing of all files, it was deemed necessary to amend the report by updating the adverse event coding (added myasthenia). The case is closed.

Write-up: 1.5 days post vax pt developed a low grade fever and felt tired. The next morning he had a temperature of 104 and chills. By Friday evening he had swollen glands, sore throat, painful to swallow.

Write-up: Pt was given a TD & Hep-A immunization on 6/22/2000 in the morning and was alright until 6/25/2000 at 15:55, she was watching TV when she starting having a seizure. Her arms and legs went outward and got stiff, her eyes went back into her eye sockets and her lips turned blue. This lasted about 7 minutes. She was incoherent and was taken to ICU and released on 6/26/2000. Put on RX for seizures. She is to be evaluated by a neurologist.

Write-up: On 7/21/00, the counselor saw him shake in bed, they weren''t sure if he had a seizure or not. He was hard to arouse but when he woke up he was alert. He was treated with Depakote.
