Diagnostic Lab Data: Tests done at hospital - CBC, CRP, ESR, Bld culture - reported as normal by hospital. Pt was kept in hospital overnight and started on clindamycin for cellulitis concerns. Pt was discharged on 3/10/16 next day.

Write-up: Pt received Tdap (R) arm, MCV (R) arm and HPV (L) arm on the 7th March 2016, around 1 PM - pt tolerated the vaccination well, was given under aseptic condition - pt left with mom. Mom called on 3-9-16 - stated had redness (R) arm couple of hrs after vaccination and on 3-9-16 developed fever 101 degrees F. Since office was closed pt was told to go to ER.

Write-up: This spontaneous report was received from a physician via other company and refers to a 4-month-old female patient. The patient was normal and she was a vaginally delivered term baby with no problems. The patient had no relevant concomitant medications or past drug history. On 29-DEC-2016 at 16:30, the patient was accidentally vaccinated with a 0.5 ml dose of GARDASIL, lot # M028323 with expiry date on 26-SEP-2018 (indication was reported as accidental administration and route of administration was reported as injection), which was intended for another, 13-year-old patient. The physician did not observe any adverse reactions after the vaccination. At the time of this report the patient was breast feeded calmly. Additional information has been requested.

Write-up: As soon as received GARDASIL vaccine, had burning thru entire body. Felt different after that. Balance issues, concentration issues. Said her brain felt different. Soon after that, had a virus that was thought to be flu. Treated with antibiotics. Had a headache that would not go away for 45 days. Went from being a 115 lb athlete, to a 155 lbs and could not eat. Hospitalized at two hospitals. Also hospitalized other times and other places afterwards. She never got better--had terrible pain. Had multiple tests and saw many doctors and all tests were normal. They could never find the cause of the headaches, but she did have 2 surgeries to improve the headaches-decompression surgery in 2015 and removal of nerves in forehead in 2016. She was diagnosed with POTS and with polycystic ovarian syndrome after vaccination. Had possible seizures-saw over 200 doctors. Died in the bathtub due to drowning.
