Write-up: Patient mother reports. Seen by RN on 12/22/2014 erythema at sight approximately 3 inches in diameter. Pt reports itching and pain to touch. Pt mother reported using hot and cold compress. Pt seen by provider on 12/22/14 at 9:57 AM and Benadryl 12.5mg/5ml was prescribed.

Write-up: Around 5:30PM, patient had chills, fever of 100F with headache and neck pain starting around 7PM. Per patient at 12:30 that night, temp was 101.4F and went to ED. Rash noted on lateral right ankle in ED.

Write-up: This is a spontaneous report from a contactable physician. A 26-year-old female patient received TRUMENBA on 20Feb2015 at 9:30 am at 0.5 ml single dose via intra-articular route. Concomitant medications were none. Medical history included asthma when she was a child, now resolved. On 20Feb2015 the patient developed high fever, headaches, and a small rash on her leg as a reaction to the vaccine. She went to the emergency department after midnight on 21Feb2015 and was admitted to the ICU to rule out meningitis since there was currently an outbreak of meningitis in the community. The patient did not have meningitis. On 21Feb2015 chest X-ray was negative for meningitis, lumbar puncture was negative and showed no WBC, cultures are still pending, multiple laboratory tests showed no signs of infection. High fever recovered on 21Feb2015. Outcome of the events headache and rash was recovering. The rash was described as minimal at the time of reporting.

Write-up: Pt has pictures of hive like rash that appeared Sunday morning 3/1/15 on her left arm, near site of injection and right leg. States rash spread over her entire leg and most of arm. Rash was gone by Sunday night. Rash reappeared the next morning, Monday, on leg and lasted for several hours and this morning, 3/3/15, as well.
